• Diploma/TOR

• NBI Clearance

• Life Insurance Certificate

 

     Medical Result:

• Drug Test                        

• Physical Examination

• Chest X-ray                   

• Complete Blood Count

•  Urinalysis                   

• Fecalysis

• Employment Certificate

• Proof of Contribution (latest 3 months) in SSS,

     Philhealth, & Pag-ibig
